Payments and Referrals


All Eye Health Services physicians are participating providers for Medicare, various vision plans, HMO’s and managed care products. Insurance coverage for eye exams varies. Some plans only cover routine well eye exams. Other plans will not pay for your exam unless you have a medical eye condition or disease. If you are not sure of your coverage for routine or medical eye care, it is best to contact your insurance carrier’s member services or your employer’s human resources department before your visit.

Payment of co-pays, deductibles and non-covered services is expected at the time of your visit. A refraction (the determination of the eye’s refractive error for the best corrective lenses to be prescribed), is a non-covered service of Medicare Part B. There is a fee for this non-covered service and is expected at the time of your visit. For Commercial, Medigap, HMO and PPO insurances, if coverage exists under these plans, claims will be submitted.

Insurance companies strictly enforce regulations regarding referrals. If your insurance plan requires a referral, please contact your primary care physician before your visit to Eye Health Services.
