Important Notice

You may use the portal to send messages to your doctor, but please be aware of the following.

  1. Messages are not checked daily. If you need an immediate response, please call our offices directly.
  2. Please use “Send a Message” when sending messages through the patient portal. For updates to your chart, please use the “Request Chart Update” under the “My Chart” tab.

Please read the Frequently Asked Questions below before proceeding to the portal.

Q. What is my username?
A. Your username will always be the email address you used during the sign up process.

Q. I forgot my password. How do I recover it?
A. Click on the “Forgot your Password” link underneath login button. You will then be prompted for your email address. After answering the security question, a one-time use link will be sent to your email. Please use that link to choose a new password.

Q. I forgot my security questions. How do I reset my password?
A. You will need to call one of our office locations and have us reset your account.

Q. The site does not load properly. What can I do?
A. You must use the latest version of either Internet Explorer or Mozilla Firefox in order to view the site properly.
